SSA Statement of Death Form with Informant Name and Address
Overview
The Statement of Death form is a Social Security Administration form with Informant Name and Address (form SSA-721) is used to verify a death so survivors of the deceased family member can apply for social security benefits.
There are two versions of this form to select from in MIS Professional:
- SSA721
- SSA721 - with Informant Name & Address
- This version of the form populates the Informant name and address details in the left box below the "Name of Deceased" field.
To run this report:
1. Navigate to Reports - Reports Selection
2. Select the "SSA721 - With Informant Name & Address" form from the list
3. Click Run
